On September 17, 2024, Hospital del Mar joins the commemoration of World Patient Safety Day, an initiative promoted by the World Health Organization (WHO) aimed at increasing awareness about safety in healthcare systems. This year, the campaign emphasizes the importance of accurate diagnoses under the motto “Right Diagnosis, Safe Patients.”
According to data provided by the WHO, diagnostic errors contribute to 16% of preventable harmful effects in healthcare, occurring in various healthcare contexts. These errors can not only have serious consequences for patients, including prolonged illnesses, disabilities, or even death, but also significantly increase the costs associated with healthcare.
Reducing diagnostic errors requires a clear understanding of the diagnostic process, which includes patient presentation, medical history, follow-up, and reassessment. This involves the participation of patients, families, caregivers, healthcare professionals, as well as industry leaders and policymakers, being essential. Each of these actors must contribute to improving the diagnostic process and have the ability to express their concerns.
Hospital del Mar’s Quality Program has implemented various strategies to increase patient safety in the diagnostic process. These include reviewing rapid diagnostic pathways and sample transportation to Pathology and laboratory, as well as improving communication of results among healthcare teams.
Hospital del Mar is also part of the Patient Safety Alliance, supporting initiatives driven by the Department of Health that include promoting hand hygiene, ensuring patient identifications, safe surgeries, and projects aimed at eradicating infections. Additionally, it has established the Commission for the Evaluation of Healthcare Activities (CA3), dedicated to learning and improving from the analysis of adverse events.
Creating a safe environment for patients is a collective responsibility that involves all healthcare system actors.
